Hi All,
An associate of David Wilcox, named Dr. Sevasti Koukouritaki, has sent this email concerning human clinical trials:

Dear Helen,

Happy New Year to you and your family!
I am sending you the updated 2010 consent forms for the GT study in case you want to send us any blood samples for analysis.

Each participant has to complete Assent (if Child) or Consent (if Adult) and HIPAA. Multiple participants from the same family can complete a single Questionaire.
Please let me know if you have any questions.

The forms are in .PDF format and can be read, and printed, with Adobe Reader. Reader is free and if you do not have it already installed on your computer, you may download it here at no charge: http://get.adobe.com/reader. As I understand it, the forms are to be printed, signed, and returned to Dr. Koukouritaki with the blood sample. Below is an email I received from Dr. Wilcox concerning shipping details, including shipping costs:

Hi Chris,

Patients have to sign Consent (adult) or Assent (for child), HIPPA and fill out family history form (Questionaire) and include in the box with blood samples that are sent to us. Most of the time, the facility collecting the blood will ship it and we will pay for the shipping cost. Most of time facility will not charge for blood draw although, if they do charge, we will pay for it. If facility does not ship, Dr. Wilcox will provide address and directions for shipping by fedex and Dr. Wilcox will pay for shipping.

Chris, Can you make sure that this information including all of the study paperwork is available when you go to the RESEARCH page on website?
